TERMS OF SERVICE

Training: GOLDEN HOME FITNESS, INC will provide training to you the following areas: Individual Personal Training, Virtual Personal Training, Group Training Classes. Specific type and frequeny TBD after your initial meeting.

REFERRALS:  Golden will provide YOU one free hour of training for each new Client you refer to Golden. This free hour will be earned when the new Client has completed his/her 1st training session.

SESSION CANCELLATION:  Conflicts or other issues may arise that require either you or your Trainer to reschedule a training session. In the event you need to cancel a scheduled session please contact your assigned Trainer directly at least 24 hours prior to that session. Make up sessions will not be provided if notice is not given. Missed sessions will expire after 60 days, as long as the Client maintains an active contract. You may suspend your account for up to 1 month after which your automatic scheduled invoicing will continue.  Longer suspensions may be allowed (up to 3 months) with a valid medical reason.

TARDINESS/NO SHOW:  Your Trainer will wait 10 minutes from the start of the scheduled training session for you arrive at your home or other agreed upon training location. If you do not arrive within the 10 minutes, and you have not called the Trainer at least 24 hours in advance to cancel the session, you will be responsible for payment of the session. If you arrive within the 10 minute time period, training will be performed for the remainder of the scheduled session.

COMMUNICATION WITH YOUR TRAINER:  At the first training session please be sure to exchange telephone #’s and email addresses with your Trainer. Please contact your Trainer directly in the event there is a need to cancel and/or reschedule a session.

TIPPING YOUR TRAINER:  Tipping your professional Golden Home Fitness trainer is NOT encouraged. Please do not tip your GHF Trainer.

SOLICITATION OF GOLDEN TRAINERS:  As a Client of Golden, you are not allowed to solicit or contract with a Golden Trainer except through Golden. By proceeding to the payment section, you agree that any violations of this clause will result in a $1000.00 administrative fee. Trainers contracted through Golden have agreed, in writing, that they are not allowed to accept direct employment or referrals from Golden Clients.

TERM:  This Agreement Term shall be for an initial 3-month period unless otherwise noted. It will be billed on a monthly basis. This initial 3-month term may be canceled if 1) the Client re-locates outside the GHF service area or 2) has a valid medical excuse.

This contract will convert automatically to a monthly term on the 4th month (same day of month as original contract) renewing monthly thereafter. This monthly term contract may be canceled if notice is given in writing (email) at least fourteen (14) days prior to the monthly renewal date. If you notify us within this 14-day period prior to billing, your invoice will be processed.  Once an invoice is processed, there are NO REFUNDS for training hours invoiced.
